Excellent work Steve, Being the deconstructionist that I am, I try to work out the elements.
Tell me if I am close.

Shot wide open at f2.8 or 3.5
If it is worked in CS3:

Adjustment layer > Gaussian blur, etched back arouns the eyes and muzzle
Duplicate layer> multiply > lasso tool to select an oval and delete to create the vignette
Adjustment layer > Black and white > maybe with tone set way low

Or some comination for the above...whatever your did, it is a great result!

Converted to greyscale and slight adjustment in curves.

Nothing else.
